Forms for Peer Leaders

Starting a New Group

Complete this form at least 4 weeks in advance of when you’d like your WoVeN group to start! List all the details for your group and any confirmed group members. You can also use this form to request recruitment supplies for your upcoming group.

Alumni Group Registration

Alumni Groups give WoVeN women the opportunity to connect beyond their 8-week groups. Once you decide on a group theme you’d like to run, visit this page to register your upcoming Alumni Group!

Certificate Request Form

This form is for requesting certificates of group completion for your group members. Complete this form at least two weeks in advance (by Meeting 6) so your group members can receive their certificates after their last meeting.

Ordering Outreach Supplies

WoVeN has outreach supplies to help spread the word! Complete this form to request materials for community events and to help with recruiting group members.

Alumni Groups

Alumni Groups are designed to be used with existing groups or with newly formed groups of WoVeN graduates nationally. Alumni Groups were developed to be intentionally flexible – you can run a single group or decide to do a series. It is up to you!

Once you decide on a group theme you want to lead, submit the registration form under Forms for Peer Leaders to register your group. After submitting the form, you will receive an email with materials for the group. You can also direct group members to the Join a WoVeN Alumni Group page so they can enroll in your group!
